
Assessments can be provided as a one time service, or as a step in the process of becoming an ongoing client.
The assessment will include the following: - interview with the individual, family, and/or care providers (caregiver assessment can be part of the evaluation) - evaluation of current health, mental health, daily functioning - typed report including diagnosis and recommendations - assessment for level of dementia & cognitive functioning - assessment of depression and suicide risk.
Current fee for this service is $150-250 (generally, I charge $150 for in office assessments, and $250 for in home assessments). Sometimes insurance can be accepted. Assessments are useful to provide mental health diagnosis, determine needs, and answer questions about a loved one.
